Release form for Intensives and workshops

Welcome to the Take it From the Top Intensive! We are committed to providing outstanding theater

training in a safe and supportive environment. Please complete this form and sign where indicated. A

parent or guardian must complete this form before the program begins.

Assumption of Risk and Release of Liability

Recognizing that participation in this program involves certain risks, and in consideration of my child

being permitted to participate in the program, I agree to the following:

1. I release Take It From The Top (TIFTT) from any and all costs, claims, injuries, or illnesses resulting from my child's participation in the program, except in cases of gross negligence or willful misconduct.

2. I agree that neither TIFTT nor any of its employees, independent contractors, directors, or officers will be held liable for any injury that may occur to my child while attending the workshop. This includes,

but is not limited to, activities such as singing, dancing, physical comedy, stagecraft, theatrical mask work, warm-up exercises, and breaks.

3. I grant TIFTT permission to contact emergency services and authorize treatment for minor injuries or medical issues. In the event of a serious injury or illness, the parent or designated emergency contact will be notified as soon as possible. However, if urgent medical attention is required, treatment may proceed before contacting the parent or guardian.

4. I release TIFTT and its respective employees, independent contractors, directors, and officers from any and all legal or financial claims related to participation in this program.

Permission for Independent Commute and Alternative Release

This release grants permission for the enrolled student to independently commute to and from the

program location. By signing this form, the parent or guardian acknowledges allowing the student to

travel independently and accepts full responsibility for their safety during the commute. TIFTT and its

staff are not liable for any incidents or accidents that may occur during the student's travel.

Media Release

Participants may be photographed or videotaped for Take It From The Top (TIFTT) promotional and

educational materials.

1. I authorize TIFTT to record the image and voice of my child and grant permission for TIFTT and its

authorized affiliates to use these materials for educational, advertising, and promotional purposes.

2. I understand that these images and recordings may be duplicated, distributed, and/or altered without

future compensation or liability.

3. I grant TIFTT permission to use these recordings in connection with the promotion of its programs.

By signing below, I acknowledge that I have read, understood, and agree to the Assumption of Risk and Release of Liability Statement, the Independent Commute and Alternative Release permissions, and

my selection regarding the Media Release. I understand that my consent is voluntary and can be revoked at any time in writing. However, revocation will not affect actions taken before receipt of the

notice.

I also acknowledge my child's role in their safety and will discuss the importance of following program

rules and behaving responsibly.
